Shipping

If you purchased a product that includes paper materials, payment must be received before Mathematica will ship materials.
 
Materials for training are generally shipped at least 1 week before training.
 
Mathematica cannot ship materials to a PO box.
 
If you cancel your order, you will be asked to return paper materials.

QCIT User Guide (If Applicable)

Sale of the downloadable QCIT User’s Guide is final. This purchase is not eligible for return, exchange, or refund.

 

QCIT Rating Form for Observations (If Applicable)

Sale of the QCIT Rating Form (download and use) is final. This purchase is not eligible for return, exchange, or refund.

 

QCIT Recertification (If Applicable)

Sale of the QCIT Recertification Exam is final. This purchase is not eligible for return, exchange, or refund. Certification expires on a person’s recertification date, and the exam period closes 16 weeks after the recertification date.

 

QCIT Observer Certification Training (OCT)  and Training of Trainers (TOT) (If Applicable)

CLIENT CANCELLATION AND TRANSFER POLICIES
All Client cancellation or requests to transfer a registration to another Training must be received in writing via email to QCIT@mathematica-mpr.com. Transfers are contingent on the availability of another training of the same type and price.

 

Cancellation and/or transfer requests received up to fifteen (15) calendar days prior to the start of a Training will receive a full refund of any registration fees paid.

 

Mathematica will allow Participants to apply full registration fees paid toward a future training scheduled within one (1) year of the original registration date.

 

Cancellation and/or transfer requests received between one (1) and fifteen (15) calendar days prior to the start of a Training will receive a refund of registration fees paid minus a cancellation fee in the amount of one hundred dollars ($100.00).

 

Customers who fail to attend, cancel, or request to transfer, without providing notice at least one (1) calendar day prior to the start of training will be charged the total registration cost.

 

A substitute is a single individual who attends the entire event in lieu of the original Participant. A substitution may be made up to (3) three days prior to the start of a Training.

 

MATHEMATICA CANCELLATION OF TRAINING POLICIES
Mathematica reserves the right to cancel or change a Training at any time and for any reason, including but not limited to, lack of participation, trainer availability, weather or natural disaster. In the event of the cancellation of a Training by Mathematica, Participants will not be charged for the Training. Mathematica will endeavor to provide notification of cancelation or changes within fifteen (15) days of a Training, whenever possible. Mathematica is not liable for any direct, or indirect, consequential or special damages that may be incurred due to a cancellation of a scheduled Training, including, but not limited to, cancellation penalties for transportation or accommodations. Client’s sole remedy shall be the refund of prepaid course fees along with waiver of any course fees that may be due but not yet paid.
