QCIT rating form: limited license to download and conduct observation(s)

Certified and provisional observers must use a QCIT rating form to conduct a QCIT observation. Observers use one rating form per observation (each rating form has space for scoring up to 7 cycles). Observers must purchase a limited license to download and use the QCIT rating form to conduct each observation. Observers should choose the exact number of rating forms needed for their observations.  For example, if you are conducting 20 observations at $4 per observation, the cost will be $80.

We are unable to provide refunds for unused observations; however, observers can come back to this site to purchase more observations as needed.

Customers will have access to the QCIT Rating Form in the Paradiso Learning Management (LMS) site for 3 months after their purchase to download and save the electronic file. Sale of the QCIT Rating Form (for download and use) is final. This purchase is not eligible for return, exchange, or refund.
